Demonstrate how to engage stakeholders in codesign process for healthcare and human services network development. Presenters will provide case study of how our organization, in partnership with a healthcare system co-led network development and established a framework for shared decision making.
Explain the value of a hub and spoke model for responding to social care referrals. Presenters will explain the evolution of the model for triaging social needs referrals as a Social Care Hub.
Identify essential components of information exchange for an effective, closed-loop social care referral. Presenters will describe the process for prioritizing essential components of a referral when information is exchanged among healthcare and social service providers.
