Enhancing care transitions and medication safety is a critical focus for CMS. Over the last three years, the New England QIN-QIO has been partnering with providers to better understand and tackle drivers of poor care transitions. Who should attend: Providers interested in learning about innovative efforts to enhance access to care for vulnerable populations and reduce unplanned hospital utilization.
The New England Quality Innovation Network-Quality Improvement Organization (QIN-QIO), in collaboration with the New England Rural Health RoundTable and the New York Rural Health Association, invites you to join us on Wednesday- January 10th (11:30am -12:30pm) to learn what three rural New England providers implemented to address their patients’ unique needs. - Evaluate how data can help inform intervention selection and implementation progress,
- Identify the role medication safety can play in transitions of care, and
- Describe key elements for successful care transitions in rural communities.
